Website only auto-fills Password?

Hello,

I’m trying to set auto-fill for this website using custom fields through the Firefox extension. However, I can only get the same password sent to each field: “Bob” in all three boxes.

I think have the field names right. What’s wrong with my setup?

I have confirmed that this issue occurs in Firefox, but not in Chrome.
