Only one has/gets my updated information. How can I get rid of the duplicate/older entry?

It’s not entirely clear what problem you’re having. One thing you might want to try is syncing your vault.

In the browser extension, you can go to Settings > Vault > and select the option to Sync vault now.
In the mobile app, you can go to Settings > Other > and select the option to Sync vault now.

The best way to get rid of a large number of unwanted entries is by logging in to the Web Vault, because unlike the other apps, the Web Vault allows you to delete multiple items at once. Simply click the check box for items that you no longer wish to keep, and then click “Delete” from the kebab menu (three dots) in the upper right corner:
