"Something went Wrong" with Duo

We are using on-premise hosting with Duo 2FA login.

There are 4 primary users, 3 use the BW app and do not have issues, but 1 user prefers the web interface. He clears his cache daily, and the next time he tries to login to Bitwarden the following happens:

  • Enter Creds
  • Request Push Notification to his cell
  • Approves the request on his cell
  • Website reports “Something Went Wrong”
  • Press Cancel button
  • Does the above process again, but with success.

Does anybody know a way around this so that he can login on the first go?
