Passwords don't appear. Message states that they are not in vault

Despite the fact that my passwords are saved in my vault under “Login”, if I try to open an app or account, I receive a message that states that the information is not located in the vault. I have to save my passwords/usernames a second time in order for it to work.

Is this in an app or the Web Vault?

It sounds like you may have invalid URLs (if any) stored in the saved items. Have you imported these from another tool? Or maybe you are missing the URL entries at all?
