In short: You have to do it all yourself, manually. And apparently your license cannot be transferred.

You might want to go and leave a vote for the feature request to make this process easier.

There are conflicting reports about whether this is the case or not (for individual Premium subscription). Have you tried contacting support to request assistance with this?
