I just discovered, after a significant amount of web searching, that you have to ENABLE App Extensions from within Bitwarden.
Open Bitwarden. Under “Auto-Fill” click App Extension. Bottom of screen click “Activate App extension”. Click “Bitwarden”. Next Screen should indicate Extension Activated. This information is not clear within the iOS app.
You also need to be sure to have selected Bitwarden to automatically fill under settings->Passwords->Password options…
